Plan Recommendation Report
Our Plan Recommendation Report offers a targeted approach to Occupational Therapy recommendations for plan reviews. While not as extensive as a full functional assessment, this streamlined service provides essential insights for participants with limited funding. It’s an efficient solution that balances cost-effectiveness with valuable therapeutic guidance.

Standard Functional Assessment
Our Standard Functional Assessment is the cornerstone of our services. This thorough evaluation includes a 2-hour in-depth assessment, pre-assessment questionnaires, and clinical testing. Our experienced Occupational Therapists conduct detailed observations, resulting in a comprehensive report that provides a solid foundation for effective therapy planning.

Supported Independent Living (SIL) Functional Assessment
Building on our Standard Functional Assessment, the SIL assessment incorporates a detailed analysis of various accommodation options. If our therapist determines SIL as the optimal choice, the report will provide a comprehensive justification, outlining all considered options to support the participant’s independent living goals.

Specialised Disability Accommodation (SDA) Functional Assessment
Our SDA Functional Assessment extends beyond the standard evaluation to address specific SDA criteria. This in-depth assessment considers various accommodation options, with a focus on specialised needs. If SDA is deemed most appropriate, our report will provide a detailed justification, addressing all relevant criteria to support the participant’s unique accommodation requirements.

Cognitive Functional Assessment
This assessment combines our Standard Functional Assessment with an additional Occupational Therapy cognitive evaluation. While not a diagnostic tool, this assessment provides valuable insights into the impact of cognitive function on daily activities. The additional 2-hour cognitive assessment enhances our understanding of the participant’s overall functional capacity.

Sensory Assessment and report
Our sensory assessment will help identify your unique sensory profile, allowing for tailored support strategies and interventions. Medicolegal Functional Assessment
Our Medicolegal Functional Assessment is designed to meet the specific requirements of legal proceedings, including tribunal matters.
