Functional Assessment Services

This is a very short report which is designed to assist with making specific Occupational Therapy recommendations for a plan review. This does not include a full functional assessment and will therefore not be as comprehensive. This is perfect for participants who have limited funding, however, please note that it may not receive as successful outcomes as more comprehensive options. 

FUNDING REQUIREMENT:

5 hours + travel

A SIL functional assessment the same as a standard functional assessment with the addition of assessing and considering various accommodation options. In the event that SIL is assessed by the therapist as the most appropriate option, all considered options will be included in the report to detail and justify Supported Independent Living for the participant. 

FUNDING REQUIREMENT:

15 hours + travel

A Cognitive functional assessment the same as a standard functional assessment with the addition of . In the event that SIL is assessed by the therapist as the an Occupational Therapy cognitive assessment. Please note that this is not a diagnostic tool, nor will it identify the level of cognitive impairment. The assessment used will take approximately 2 additional hours to complete on top of the original assessment and will assist in understanding the impact of reduced cognition on function. 

FUNDING REQUIREMENT:

16 hours + travel

This is the most common form of functional assessment. It includes a comprehensive 2 hour assessment, including observations by the occupational therapist, questionnaires completed prior to the assessment and clinical testing, followed by a very detailed report.

FUNDING REQUIREMENT:

14 hours + travel

An SDA functional assessment the same as a standard functional assessment with the addition of assessing and considering various accommodation options and addressing the detailed and specific SDA criteria. In the event that SDA is assessed by the therapist as the most appropriate option, the criteria will be addressed and all considered options will be included in the report to detail and justify Specialised Disability Accommodation for the participant. 

FUNDING REQUIREMENT:

18 hours + travel

A medicolegal functional assessment is a very specific assessment requested by a legal team assisting with matters at tribunal. The purpose of a medicolegal functional assessment is for the assessor to be an independent third party to provide a non-biased clinical recommendation based on the request of the legal teams.
